Decision Registry. The authoritative, append-only home for consequential decisions: what was requested, who or what acted, which evidence and policy applied, which approval or denial occurred, what execution path was used, and what observed outcome followed. A decision registry owns decisions the way a ledger owns money movement: logs may describe it, but they do not become it.
The registry is different from a single audit record. An audit record is the durable account of one operation; the decision registry is the governed source of record that keeps those records together, queryable, replayable, and distinct from lossy observation. In AISDR, the decision registry is the Truth Plane home for governed AI/security action.
The lossy telemetry boundary is what keeps the registry honest: telemetry can describe a decision, but it must not become the place the decision is recovered from.
